Timber Creek Total Care Program Overview

Timber Creek Total Care operates as a 24-hour Therapeutic Group Home. Our facility accommodates 20 beds and is specifically tailored to provide treatment for children and adolescents aged 13 to 18 who have been diagnosed with psychiatric disorders. Our Therapeutic Group Home offers specialized medical, psychiatric, psychosocial, and behavioral treatment services aimed at helping youth and their families acquire the skills needed to exhibit mental wellness and achieve positive community integration.

We cater to youth who exhibit a history of serious problem behaviors characterized by persistent, unpredictable symptoms that may endanger the health and safety of others. These symptoms encompass severe impulsivity, mood swings, explosive outbursts, self-harm tendencies, or assaultive behavior. Furthermore, these children and adolescents have experienced previous failures in less intensive treatment settings – although they do not necessitate the level of care provided by an acute psychiatric hospital, they still require a supervised, secure environment and specialized therapies. Our objective is to facilitate each youth’s swift progression to a less restrictive level of care. Admission prerequisites comprise a psychiatric diagnosis and a referral from a psychiatrist, physician, or licensed psychologist.

Situated in a safe and secure environment, our Therapeutic Group Home is specifically designed to cater to intensely disturbed youth. Equally crucial is its role in fostering positive self-esteem and age-appropriate behavior through the implementation of a therapeutic milieu. The involvement of youth, families, guardians, and referring or prospective service agencies is integral to the ongoing multidisciplinary treatment process.

Timber Creek Total Care is well-prepared to handle any psychiatric or medical emergency promptly, thanks to our close relationship with acute care programs.

Timber Creek Total Care’s multidisciplinary team, led by our Therapeutic Group Home Clinical Director, offers personalized treatment. We’re creating a therapeutic environment formulated under the guidance of the Clinical Director and executed by all Treatment Team members. Our team comprises registered nurses, licensed practical nurses, Certified Nurse Aides, Medication Aides, social workers/therapists, and Direct Support Staff. Additional professionals, such as those in Pharmacy and Dietary Services, contribute to the treatment process.

Family or legal guardian involvement is pivotal in youth treatment. Families are expected to participate in family therapy throughout the treatment journey. We believe in providing treatment within the familial context whenever feasible, emphasizing active participation of both family and youth in the multidisciplinary treatment planning. Recognizing these youth will eventually return to their communities, we adopt a holistic approach that incorporates the individual, their family, and their community throughout all treatment phases.

All youth will actively participate in comprehensive treatment, which includes individual therapy, group therapy, family therapy, community meetings, medication management, psycho-educational therapy, and therapeutic outings and passes. Additionally, our program will utilize a privilege-level economy system which is intended to be used as a skill-building process through which youth are supervised and guided by our staff.

As youth demonstrate acceptable behaviors in the social and interpersonal skill areas, adult supervision and needed therapeutic interactions will diminish in intensity and frequency. Residents will receive positive and negative consequences for their behaviors to motivate them.

The privilege-level economy system is regularly adjusted to address developmental needs and intellectual functioning, with ongoing monitoring by the Treatment Team. We will integrate the Boystown Model into our approach, further enhancing the effectiveness of our therapeutic interventions.

Timber Creek Total Care prioritizes diminishing the severity of behavioral health issues identified as reasons for admission. Residential treatment centers, conversely, emphasize offering a comprehensive array of services to tackle individuals’ behavioral health concerns.

Beyond addressing immediate needs and symptoms, our focus extends to fostering long-term mental well-being and successful integration into the community. With a foundation built on safety, compassion, and individualized care, we strive to empower each young person to overcome challenges, realize their potential, and embark on a path to a brighter future.

A Team Approach

Our Team, under the direction of the Clinical Director, provides kids with individualized treatment and coping skills to help them become successful and happy in their lives. Youth will actively engage in individual therapy, group therapy, family therapy, community meetings, medication managements as prescribed by the Psychiatrist, psycho-educational therapy, and therapeutic outings and passes.

Education

Residents attend school Monday through Friday at Omaha Street School or Blackburn Elementary.

In our therapeutic group home, we provide individual, group, and family psychotherapy, educational services, social skills building, life skills teaching, goal setting, recreational or leisure activities, therapeutic teaching interactions, and therapeutic outings and passes. Throughout each day, our staff actively engages with the youth in teaching interactions, primarily focusing on the appropriate use of social skills and addressing problematic behaviors when they arise. Youth in our program progress through a series of levels defined by the motivation system. Advancement from one level to the next occurs as youth demonstrate treatment progress and meet specific criteria.
